Welcome to Disability Services at the University of Wisconsin-Fox Valley. Students inquiring about special services should contact the Student Services office for an initial packet.

Documentation

Students must provide documentation stating the nature of the disability and eligibility for services prior to receiving accommodations. Most often documentation will be obtained from a qualified professional (physician, psychologist or psychiatrist).

Accommodation Plans

When the initial packet and sufficient documentation have been submitted the coordinator of disability services will provide a recommended accommodation plan. A copy of this plan will be submitted to the student as well as to the student's professors. Students are encouraged to set up a meeting with each faculty member involved. This meeting should take place early in the semester.

The Disability Services Staff

Brian Schultz is the coordinator of Disability Services. Brian works out of the UW Colleges Central office in Madison. Brian reviews student files and sets up the recommended accommodation plans. Tina Koch is the contact person for Disability Services at the UW-Fox Valley campus. Tina’s office is located in the Student Services. Her role is to assist students on campus with their needed accommodations.
